Care Coordination

BCHD has partnered with Healthcare Access Maryland (HCAM) since 1997 to serve as the designated Medicaid care coordination entity for Baltimore City and the Administrative Care Coordination Unit/Ombudsman Program. The Care Coordination Program (CCP) is an outreach program that provides short-term care coordination services to Baltimore City residents. The Care Coordination Program assists Medicaid/HealthChoice eligible beneficiaries in accessing and using their Medicaid benefits. Most importantly, CCP links clients to the Maryland’s Managed Care Program, HealthChoice. The Care Coordination Program educate clients about health benefits, linkage to primary care and specialty services, work to improve birth outcomes in Baltimore City with innovative prenatal outreach strategies and strive to protect our city’s most vulnerable populations by providing access to health services and community resources.

HCAM annually serves more than 5,000 pregnant and postpartum people supported through Medicaid through the B’more for Healthy Babies centralized intake system.
